A Program For Post-Stroke

What is the Reach Program?

The Foothill College REACH Program is designed to provide the essential link between outpatient rehabilitation and full community reintegration to adults recovering from stroke.

With the assistance of a counselor, students participate in individual goal setting. Classes are tailored to meet specific needs allowing each student to reach their maximum physical, emotional and social potential and to achieve the highest level of independence.

Who can enroll?

Admission to the REACH Program is open to any resident of California. It is appropriate for post-stroke individuals, who have left the hospital or the rehabilitation center, as well as for those who have old limitations but need guidance and support in learning new activities.

Classes for family caregivers are also available.

What classes are taught?

Classes are taught by instructors who are specialists in Physical Therapy, Occupational Therapy, Speech/Language Pathology and Counseling in the following areas:

  • Mobility and Remedial Fitness
  • Gait training
  • Mobility, balance and ambulation
  • Muscle retraining, strengthening
  • Stress management
  • Pain management
  • Independent Living Skills
  • Perceptual/motor assessment
  • Activities of daily living
  • Exercise of upper extremities
  • Speech and Language Skills
  • Communication skills
  • Memory and cognitive training
  • Emergency procedures
  • Life Adjustment Counseling
  • Orientation to the program
  • Values clarification
  • Adjustment to disability
  • Goal setting
  • Community re-entry
  • Family counseling

Where is the Reach Program located?

The Foothill College REACH Program is located at the Middlefield Campus, Building P, 4000 Middlefield Road, Palo Alto, CA 94303.

How do I enroll?
