Fine Arts Facility Rentals Application

This rental application is for renting Smithwick Theatre, Lohman Theatre, Appreciation Hall, and Fine Arts classrooms, only. For all other locations, see the General Facility Rentals website.

All facilities and services must be specified in the application, which will be the basis for preparing the contract. A non-refundable deposit of $500 is required to reserve facility and must be received within ten days after the application has been submitted.

Please note that the interactive procces for creating Rental Contract form has changed slightly! You (the client) will now be creating the actual contract document on your side, which is why it is important to print, sign, scan, and e-mail back! Thank you!

Read carefully and agree to the following Fine Arts Conditions:

  • Food Services- Any food or beverages served must be from a licensed caterer of Santa Clara County.
  • No food may be cooked on campus. No food or beverages other than water are allowed inside any Fine Arts classrooms or theatres.
  • Technical Requests- All technical requests must be made at least 30 days prior to the event. The Facilities Coordinator will make final decisions on all technical requests. No outside equipment may be used that the Facilities Coordinator deems to be unsafe or uses power beyond allowed electrical capacity. Only District personnel may operate District equipment.
  • Facilities may not be filled beyond permitted capacity. Only performers, crew, and District-authorized personnel are allowed backstage. Doors, hallways, and aisles may not be obstructed by equipment or people.
  • The client will be represented by only one person, the contract signer, during rentals.
  • Specialized personnel may work with District technical personnel by prior, written communication.
  • Flame/fire is not allowed on or in any District property. Nothing may be attached to exterior or interior walls, signs, or posts.
  • Internet access is available only for District staff and students. Outside visitors may not use access codes of District staff and students.


  1. Agreement may be cancelled at any time prior to use should facilities be needed for college purposes.

  2. Renter shall be responsible for any damages sustained by the school premises, furniture or equipment, because of occupancy of said premises by Renter. Renter responsible for own cleanup or additional charge will be assessed.

  3. Renter shall abide by and enforce the rules, regulations and policies of the Foothill-De Anza Community College District governing board use of facilities. (

  4. Renter agrees to hold the Foothill-De Anza Community District, its Governing Board, the individual members thereof, and all District officers, agents, and employees free and harmless from any loss, damage, liability, cost or expense that may arise during or be caused in any way by such use or occupancy or college property. Per Education Code Sections 82537/82542: Public use of District property shall not interfere with scheduled instructional programs or other activities of the District on behalf of students.

  5. Alcoholic beverages and self-prepared foods for public consumption are prohibited.

The use of College facilities is subject to all conditions, including all rules and regulations governing the facilities, imposed by the College, and in addition the Renter agrees to guarantee the College against all loss or damage to the rented premises or to the building or to the fittings or furnishings thereof during the time that said facilities are used or are under control of the Renter. In this respect, the Renter agrees to furnish a Certificate of Insurance in the amount of $1 million listing Foothill College as additionally insured. Said insurance shall provide primary coverage regardless of other policies Renter may have, and shall state that the same may not be altered or canceled to the College's detriment without 30 days prior written notice to College. Renter agrees to provide College in advance of occupancy with certificates showing such insurance to be in force. Renter hereby accepts contract and agrees to be bound by terms and conditions herein.
