COVID-19 Health & Safety

Behavioral Expectations

The following safety behavioral expectations are meant to protect our students, faculty, staff, and the surrounding community.

In accordance with Board Policy 3507, the Foothill-De Anza Community College District requires that all students and employees who access on-campus programs in person be fully vaccinated against COVID-19 unless approved as exempt due to verified medical or religious reasons. 

The FHDA College Community Compact 

I understand that my ability to enter and remain on-campus as a student is a condition of my adherence to the FHDA College Community Compact, Behavioral Expectations, and Health and Safety Guidelines. 

I acknowledge and agree that these guidelines may change as on-campus and public health guidance across the country evolve and new testing and tracing methods emerge.

 

I Will Protect Myself and the FHDA community

  • I will practice good hygiene, including frequent hand-washing with soap and water and/or the use of hand sanitizer.
  • I will wear a KN95 or N95 face mask at all times while on campus, both indoors and outdoors, even if I have been fully vaccinated
  • I will report a positive COVID test to the COVID team.
  • I will stay home and test if I experience any of the following symptoms: fever, cough, fatigue, difficulty breathing, chills, muscle/body aches, headache, sore throat, and/or loss of taste or smell.

Pledge of Commitment

I agree to the expectations outlined in the COVID-19 Student Health Agreement in support of in-person classes at Foothill College and/or De Anza College for the current school year.

As required by Board Policy 3507, I understand that I must be fully vaccinated with a booster (if eligible) to access on-campus in-person services.

I also understand my decisions and behaviors impact the health and safety of all members of the community, including my fellow students, employees of FHDA, and my well-being.

I understand I may be asked to leave school and face disciplinary consequences if it is determined that I did not uphold the health and safety guidelines and expectations outlined above.

Consequences for Violations

Any alleged violation of the Compact or the Health and Safety behavioral expectations listed above will be reviewed by the Student Discipline Officer of each campus, who will have the authority to discipline or remove a student from campus in accordance with the Foothill-De Anza Student Code of Conduct (Administrative Procedure 5520) and the California Education Code Sections 66017, 76020 and/or Sections 76030-76037.  Students cited for failing to abide by campus, county, or state health and safety protocols are subject to the full range of disciplinary actions, including restricted access to campus facilities, disciplinary probation, deferred suspension, suspension, and expulsion. The Student Code of Conduct and its complete disciplinary actions apply to all students.

Evolving Conditions

Recognizing that the COVID-19 situation is evolving rapidly, Foothill-De Anza Community College District may have to adjust guidelines quickly. Updates will be e-mailed directly to all students and parents (if applicable) if there is a significant protocol change.

Health and Safety Guidelines

FHDA is committed to the health, safety, and well-being of our students, educators, families, and the general community. Reopening our school safely will be dependent on all of these constituents' cooperation and will require vigilance from every single community member both when they are at school and when they are at home. Being responsible and caring for our community is critical, and every one of us plays a vital role in supporting the health and safety of each other.
