Student Health Records

All student health records from the Foothill College Health Center for services provided prior to June 30, 2017 are available through the service provider — Planned Parenthood of Mountain View.

How to Access Records

Please contact Angelica Dumas at 650.947.4327 or Angelica_Dumas@ppmarmonte.org.

Records may only be picked up in person, with a photo ID, at the Planned Parenthood Health Center at 225 San Antonio Road. See location directions.

Hours include Monday, Wednesday and Friday, 8:30 a.m. - 5 p.m., and Tuesday and Thursday, 8:30 a.m. - 7 p.m.
