What to Bring
Please bring your insurance card (or digital equivalent) if you have one; if not, you'll just need to fill out a vaccine voucher form.- You will be asked to fill out a short Vaccine Administration Record form (name, address, physician, insurer, etc.) so that Walgreens can send the vaccination information to your primary care provider.
- You may complete and print the form to bring with you. If not, there will be forms available on site.
